Care coordination. Wellness coaching. Disease education. An empathetic ear.
Thousands of times a day, Evolent’s care management teams touch the lives of patients across the country. Working on behalf of our provider and health system partners, they help people facing major challenges—multiple chronic diseases, recent hospitalizations and terminal illnesses—to better manage their health and enjoy higher quality of life. They also coordinate care for some of the most vulnerable members of their communities, such as children in foster care and refugees.
These Care Advisors don’t work alone. They stand on the shoulders of other Evolent experts who have built sophisticated systems to identify patients who would benefit most from care management and then deliver interventions most likely to help them.
Yet, even with this team on their side, there are cases that demand extra doses of resourcefulness, grit and resolve. Care Advisors help patients navigate not only the complicated health care system, but also situations like thorny family dynamics, government red tape and company drug programs.
To navigate these complex and sensitive challenges our Care Advisors form deep and trusting relationships with the patients and their families. I’m excited to introduce a new Evolent series, The Heart of Care, that highlights these inspiring stories and what motivates our Care Advisors to serve. Whether they’re on the phone with caregivers or sitting next to patients in a clinic, these care team members gain truly incredible perspective on the struggles that patients endure in our health care system every day, and what it takes to overcome them.
Evolent as a whole remains strongly committed to addressing those challenges. Our employees share a passion to serve our communities by changing the way health care is delivered, and Care Advisors contribute significantly as genuine, resourceful connections at the point, and at the heart, of care. I’m personally grateful for their impact, and I hope these regular Heart of Care stories leave you feeling inspired, too.
