Since 2015, incentive payments from Medicare have helped providers make the switch to value-based care, and succeed in it. Practices have used these advanced alternative model payments to invest in population health, care coordination, patient engagement and more. However, without congressional action to extend these incentives beyond this year, we risk losing momentum.
