Medication Therapy Management Our program is designed to improve the quality of care and wellness of members by optimizing therapeutic outcomes OUR SOLUTIONS We operate our own Medication Therapy Management (MTM) program in-house and utilize both pharmacy and medical claims data to drive member identification and targeting for Comprehensive Medicare Reviews (CMR) and Targeted Medication Reviews (TMR). Combining technology with a high-touch outreach program, the Identifi® MTM solution provides all supporting workflows for CMR, TMR, pharmacist interventions and communication with patients and providers. Our dedicated MTM team partners directly with clients to review data on MTM identification, CMR completion rates, recommendations and results to integrate the MTM program with other care management or population health initiatives. Achieving health goals for members >95% of members who completed a CMR with an Evolent pharmacist agreed that the pharmacist provided them with education that will help them achieve their health goals.
